Please make sure to cover (mask) some of the information on your identity verification document with plain paper or other material before uploading it. If the relevant information is not covered, the document will be discarded and you will not be able to apply. Click here for information that needs to be masked for each document.

Please upload your identity verification documents as image files when you apply.
If there are any problems with the image, you will be asked to upload it again, so please check the following before applying.

Image upload procedure

STEP 1 Take a picture of your ID

Please take a picture of your ID with a digital camera or smartphone and save it to your computer (smartphone). When doing so, please select an image where the photo and written information can be clearly read. Please make sure to include the entire ID in the photo.

STEP 2 Upload a photo of yourself

  • ・Click the "Select Photo" button.
  • ・When the upload screen opens, check the notes and click the "Select photo" button to upload the image you have taken.
  • ・ Check the uploaded image and click the "Confirm" button to complete the upload.

Your identity verification documents

Please prepare one of the following items (1) to (9) and the appropriate identification document.

  • ※Please have the page with your current address written on it ready.
  • ※If your current address is different from the address on the confirmation documents below, or if there is no address space on the confirmation documents, you will need to provide a certificate of residence or a utility bill addressed to you with your current address written on it (issued within the last 3 months).

(1) Driver's license

  • 1.A document with your photo and current address
  • 2.Must be within the validity period
  • 3.Issued by the Prefectural Public Safety Commission (excluding international driving permits)
  • ※If you have a driver's license, a photo of your face is required. If the photo is unclear or your face is partially or completely hidden and cannot be confirmed, you will be asked to send it again.
  • ※When uploading the back side, please make sure to cover (mask) the "Organ Donation Intention" section with plain paper or similar.
    If the relevant areas are not covered, the document will be discarded and you will not be able to apply.

(3) My Number Card (Individual Number Card)

  • 1.The name and address side of your My Number card (front of the card)
  • ※The side with your My Number printed on it (the back of the card) cannot be used. If you send it to us, it will be discarded.
  • ※Please cover (mask) the "gender" and "organ donation intention" written on the front of the card with plain paper or other material before sending it. If it is sent with the information still visible, it will be discarded.
  • ※The notification card cannot be used as identification.

(5) Rehabilitation Handbook

  • 1.Must have the date of issue stated or be within the validity period
  • 2.A document with your photo, date of birth, and current address
  • ※Please make sure to cover (mask) the "Name of disability (type of disability)", "Level of disability", and "Gender" with plain paper or other material before uploading. If the relevant sections are not covered, the document will be discarded and you will not be able to apply.

(6) Health and Welfare Certificate for Persons with Mental Disabilities

  • 1.Must have the date of issue stated or be within the validity period
  • 2.A document with your photo, date of birth, and current address
  • ※Please make sure to cover (mask) the "Name of disability (type of disability)", "Level of disability", and "Gender" with plain paper or other material before uploading. If the relevant sections are not covered, the document will be discarded and you will not be able to apply.

If you are a foreign national and would like to choose installment payment when purchasing a device, your period of stay must be within the installment payment period. Therefore, please prepare one of the following identity verification documents: (8), (9), or (10).

(7) Special permanent resident certificate

  • 1.Must be within the validity period
  • 2.A document with your photo, name, date of birth, and current address
  • ※Back side: Required even if there is no change history
  • ※Please make sure to cover (mask) the "Gender" field with plain paper or other material before uploading. If the relevant section is not covered, the document will be discarded and you will not be able to apply.

(8) Residence Card (Qualification: Permanent Resident / Highly Skilled Professional No. 2)

  • 1.A document with your photo, name, date of birth, and current address
  • 2.The face photo and the subject match
  • ※Back side: Required even if there is no change history
  • ※Please make sure to cover (mask) the "Gender" field with plain paper or other material before uploading. If the relevant section is not covered, the document will be discarded and you will not be able to apply.

(9) Residence card (status: other than permanent resident/highly skilled professional No. 2) + foreign passport

  • 1.A document with your photo, name, date of birth, and current address
  • 2.The face photo and the subject match
  • 3.Within the period of validity of the status of residence
  • 4.With 90 days or more until the period of residence expires
  • ※Back side: Required even if there is no change history
  • ※Foreign passports are also eligible if they are still valid.
  • ※Please make sure to cover (mask) the "Gender" field with plain paper or other material before uploading. If the relevant section is not covered, the document will be discarded and you will not be able to apply.

If you apply to purchase a device in installments, the period of stay required will correspond to the number of payments.

  • ・For 24 installments, 24 months or more
  • ・36 installments for 38 months or more
  • ・50 months or more for 48 installments
