    For family members living together

    Please prepare identification documents for each contract holder that can verify that they have the same address.

    For family members living apart

    One of the following documents is required:

    • If the parties have the same surname, only "1. Identification documents for each contract holder" will be accepted.
    • If you have different surnames, we will accept "2. Documents that verify you are family members" or "3. Eligibility confirmation document with the names of each family member on one page."

  • Documented verification of same-sex partnership

    These include certificates, copies of affidavits, and receipts issued by each local government, which include the following conditions:

    • The name of the person in question matches the information in the application
    • If your local government issues a receipt, you will also need one

  • Confirmation by qualification confirmation letter

    The conditions for the qualification confirmation document to be used as family verification document are as follows:

    • The names of your family members must be listed
    • The date of issue must be stated
    • If there is an expiration date, it must be within the expiration date.

    Important points to note when uploading

    • Please submit the form with the "Insurer Number," the insured person's "Symbol, Number, and Branch Number," "Gender," and "QR Code" hidden so that they cannot be read.
