List of identity verification documents
Driver’s license	●A valid driver's license issued by your Public Safety Commission with a photo and current address. (International driver's licenses are not accepted.) ●If the address on your identity verification document differs from your current address, you will need to provide supporting documents such as a Certificate of Residence or a utility bill addressed to you with your current address (issued within the last three months).
Japanese passport	●A valid document issued by the Japanese government that includes your photo, name, date of birth, and current address. ●If the address on your identity verification document differs from your current address, or if the document does not include an address field, you will need to provide supporting documents such as a Certificate of Residence or a utility bill addressed to you with your current address (issued within the last three months).
Individual Number Card (My Number Card)	●A valid document with your photo, name, date of birth, and current address. ●If the address on your identity verification document differs from your current address, you will need supporting documents such as a Certificate of Residence or a utility bill addressed to you with your current address (issued within the last three months). ●Individual Number Card notification cards cannot be accepted as identity verification documents (nor as supporting documents). Physical disability certificate Rehabilitation certificate Mental disability certificate	●Must show your name, date of birth, and current address. ●If an expiration date is indicated, it must not be expired. ●If there is a space for a photo, a photo must be affixed. ●If the address on your identity verification document differs from your current address, you will need supporting documents such as a Certificate of Residence or a utility bill addressed to you with your current address (issued within the last three months).
Residence card + foreign passport or Special permanent resident certificate ・If your residence status is “Permanent Resident / Highly Skilled Professional Type 2”: Residence Card ・If your residence status is "Special Permanent Resident": Special Permanent Resident Certificate ・For residence status other than those listed above: Residence card + foreign passport	●Documents must include a photo, full name, date of birth, and current address. If an expiration date is indicated, it must not be expired. ●Individuals without a valid residence status will not be able to apply. ●If you are applying for an installment purchase contract or an individual credit purchase brokerage contract, you will need the following identity verification documents. ●If your current address differs from the one on the identity verification documents listed on the left, you will need supporting documents such as a Certificate of Residence or a utility bill addressed to you with your current address (issued within the last three months). ●If your period of stay does not meet the installment contract period or our company’s standards, you will not be able to enter into an installment purchase contract or an individual credit purchase brokerage contract. ●If you are canceling your contract when you return to your home country and are unable to bring your residence card with you because you have already returned it, your foreign-issued passport will be sufficient.

※Supporting documents refer to the following. For model changes, information updates, or cancellations, supporting documents are not required.
However, all documents must be issued within the last three months.

・Utility bill receipts (electricity, gas, water, or NHK reception fee) or printed documents issued by government agencies
※Please bring an identity verification document that can verify the user's age, such as a student ID or handbook.

If the contract holder is a corporation

If the contract holder is a minor

Any of the above identification documents + Our company's parental consent form + Identification document of parental authority (one of the above. However, supporting documents are not required)	●The parental consent form must be completed and stamped by the parent or guardian personally. ●In principle, if you are between 12 and 16 years old and are applying for an individual credit purchase brokerage contract (installment sales contract), payment must be made using a bank account or credit card in your legal guardian’s name.

How to use the Online Application (Consent Form)

●If the contract holder is a minor residing in a child welfare facility and the person acting as their guardian is a facility official, the following identity verification documents are required.

Required documents
●Identity verification documents for minor residents of a child care facility ●Consent by a facility staff member (please use the legal representative consent form (parental consent form)). ●Identity verification documents of the facility staff member, and documents that can verify their affiliation with the facility (e.g., employee ID). ●Documents that can verify the relationship between the minor resident and the facility staff member, as well as the contract address.

Required documents

●Identity verification documents for minor residents of a child care facility

●Identity verification documents of the facility staff member, and documents that can verify their affiliation with the facility (e.g., employee ID).
●Documents that can verify the relationship between the minor resident and the facility staff member, as well as the contract address.

※The legal representative consent form (parental consent form) must be completed and stamped by the parent or guardian themselves.
※Please complete and stamp the original legal representative consent form (parental consent form) and bring it with you.

If a representative visits the store

If the contract holder (or, in the case of a corporation, the representative or person in charge) cannot visit in person for any procedures, a power of attorney form is required.
Please also submit the representative’s identity verification documents (and, for individual contracts, a document verifying the family relationship).

Family verification documents

Family verification documents are documents that prove your relationship with the contract holder.
A document that verifies the same surname or a single document that can prove your relationship with the contract holder.

Please bring one of the following:

●Identity documents for each person that verify the same surname
●Evidence of your relationship with the contract holder
(Documents such as a Certificate of Resident Registration Details, Certificate of Eligibility, Resident Card, or Certified (Extract) Family Register, which list the names of both the contract holder and the agent. However, a Certificate of Resident Registration Details, Resident Card, or Certified (Extract) Family Register must be issued within the last three months.)
●Documents proving a same-sex partnership.
