If it is difficult for the contract holder (or representative or person in charge in the case of a corporation) to visit the branch to carry out various procedures, a power of attorney is required for the procedure.
Please also submit identification documents for the representative (including family identification documents in the case of an individual's line).
- "Family verification documents" are documents that can verify that you are family, such as a Certificate of Resident Registration, a Certificate of Registered Information, or a document proving a same-sex partnership (however, Certificates of Resident Registration and Certificate of Registered Information must be issued within the last three months).
- The power of attorney must be completed completely by the contract holder in his/her own handwriting and must be stamped with a seal (signature is acceptable).
- If any information is missing, the procedure cannot be completed. We may contact the contract holder in order to proceed with the procedure.
- If you have multiple phone numbers for which you wish to process the request, you will also need the same number of power of attorney forms.
